RESEARCH, TEACHING, AND CLINICAL CONTRIBUTIONS:

 

NARRATIVE REPORT

 

Research: Since attending medical school, I have been involved in research. My major contributions in this area are:

·        Establishing the mode of action of abortive antimigraine medications on arteriovenous anastomoses, which paved the way for the introduction of sumatriptan

·        Establishing an effect of ergotamine on prostaglandin synthesis at a time when prostaglandins were thought to play a major role in migraine pathogenesis

·        Establishing guidelines for the conservative and surgical management of os odontoideum, a congenital anomaly of the occipitocervical junction

·        Describing hemicrania continua as a chronic headache condition with an absolute response to preventive treatment with indomethacin

·        Establishing the efficacy of high-dose verapamil in the preventive treatment of cluster headache

·        Questioning the validity of the International Headache Society classification of headache by showing considerable overlap in the diagnosis of migraine and episodic tension-type headache

·        Establishing the (modest) efficacy of hypnotherapy in chronic tension-type headache

·        Describing a different sequence of events preceding the occurrence of migraine headache, depending on the time of onset of the headache

·        Describing the presentation and development (out of migraine and episodic tension-type headache) of chronic daily headache

·        Establishing that the presentation of chronic daily headache is the same, whether the initial headaches are episodic tension-type headache or migraine

·        Introducing the concept of circumstances of onset of chronic headache and describing these circumstances

 

Over the last 20 years, I have also been involved as a principal investigator in 62 clinical trials with the following compounds: acetaminophen/codeine, almotriptan, alniditan, aspirin, aspirin/acetaminophen/caffeine, avitriptan, BMS-214778, botulinum toxin, buspirone, butorphanol, dihydroergotamine, divalproex sodium, eletriptan, flunarizine, frovatriptan, hydroxyzine, ibuprofen, naproxen sodium, naratriptan, nefazodone, pizotifen, PNU-101387, PNU-170413, PNU-142633, rizatriptan, SB-220543, sumatriptan, and zolmitriptan.

 

On the basis of the above research, I have published 40 original articles of which 31 as first or second author. In addition, I have reviewed articles for Cephalalgia, Clinical Journal of Psychiatry, Headache, Headache Quarterly, Journal Watch Neurology, Neurology, New England Journal of Medicine, Pain, Pediatrics, and Southern Medical Journal.

 

Teaching: I have also been involved in teaching since I was in medical school. My major contributions in this area are:

·        Teaching experimental pharmacology to medical students when I was a research and teaching assistant at the Department of Pharmacology of the Erasmus University

·        Teaching neurology to medical students when I was a resident in neurology at the University Hospital Dijkzigt in Rotterdam

·        Teaching headache to Tufts medical students, interns at The Faulkner Hospital, residents in medicine at The Faulkner Hospital and in neurology at New England Medical Center, and fellows in headache management at The Headache Research Foundation/John R. Graham Headache Centre when I was the director of the latter institution and assistant professor of neurology at Tufts University School of Medicine

·        Teaching headache at primary care medicine and neurology conferences at Brigham and Women’s Hospital since I have been affiliated with Brigham and Women’s Hospital and a lecturer in neurology at Harvard Medical School

·        Teaching migraine pharmacology to second year medical students at Tufts University School of Medicine

·        Lecturing at primary care medicine and neurology postgraduate courses organized by the Harvard Medical School

·        Lecturing at postgraduate courses organized by the American Headache Society, formerly called American Association for the Study of Headache, Headache Cooperative, formerly Consortium, of New England, and Diamond Headache Clinic Research and Educational Foundation

Related to my teaching activities, I have given 548 lectures, posters, and workshops at local, regional, national, and international meetings in the North and South America, Europe, and Australia. In addition, I have published 23 proceeding papers and 26 reviews, book chapters, and editorials. I trained four internists as fellows in headache management, three of whom are still active in headache practice, two at the Spaulding Rehabilitation Hospital, one as director of the Pain Inpatient Unit, and one at the Michigan Head Pain and Neurological Institute in Ann Arbor.

 

Since joining Harvard Medical School in 1990, I have been actively involved in the Harvard teaching program, in particular at a postgraduate level. My involvement has been predominantly with the neurology and primary care/internal medicine programs. I have taught every two months at lunch conferences for the house staff at Brigham and Women’s Hospital (attendance: 5 – 30). I have also taught at similar conferences at Beth Israel Hospital and Harvard Community Health Plan. I have presented Grand Rounds at several of the Harvard affiliated hospitals. With regard to the postgraduate teaching programs of Harvard Medical School, I used to have my own three-day headache course, which I directed together with Martin A Samuels, MD (attendance: 30 – 50). Furthermore, I have been a regular participant, giving lectures and workshops, in the following courses: Intensive Review of Neurology, Neurological Aspects of Psychiatric Practice, Neurology for the Non-neurologist, Neurology for Psychiatrists, Office Practice of Primary Care Medicine, Practical Treatment of Neurologic Disorders, Primary Care for Subspecialists, and Review of Geriatric Neurology (attendance: 100 – 400). 

 

Patient care: I have been involved in patient care since 1980 and have contributed to the medical literature with 60 clinical communications. In 1986, I succeeded John R Graham, MD, MACP, a world-renown headache specialist, as director of The Headache Research Foundation and established the John R Graham Headache Centre at Faulkner Hospital. I was the director until 1990 and during that time increased the patient volume at the Centre from 2,500 to 12,000 visits per year. A survey conducted at the Centre by Simmons College revealed that under the program that I headed, one-third of the patients improved more than 75%, one-third between 25 and 75%, and one-third less than 25%.

Over the years I have received funding for my educational and research activities from the Erasmus University Foundation, The Headache Research Foundation, and several pharmaceutical companies.

 

REPORT OF CURRENT RESEARCH

 

Apart from continuing phase II through IV clinical trial research with a variety of compounds, I am presently conducting a large study establishing a 3-D approach to the classification of headache, including presentation, development, and causation. The basis of the study consists of a large computerized database of 1,373 patients who were personally seen by me for the evaluation of headache. Of the patients, 77% are women and 23% men; the age at the time of consultation ranged from 5 to 89 years with a mean of 40.2.

 

REPORT OF CLINICAL ACTIVITIES

 

I am in full-time clinical practice, dedicated to headache management, and see 50 – 60 patients per week. I practice at a tertiary level with many patients referred to me by internists and neurologists. The patients I see are on the average 40 years of age and have suffered from headaches for an average of 15 – 20 years. Half of them have daily headaches and 95% experience severe headaches, affecting their personal and professional lives. The patients are referred to me predominantly from the New England states but also from Canada, South America, Europe, and the Middle East.
